Twice when my lady friend shot this gun

All OEM Glock except for the frame. Did she limp-wrist it? This gun has not had this happen while I was shooting it but if it has I don't remember

If the malfunction happens only when she's shooting it, then it's reasonable to assume it's a "limp-wrist" situation.

Usually limp-wristing can be observed. Did you watch her hands / grip / recoil management while she was shooting?
 
Classic limp wrist! No question. She needs help with her grip at the very least. Mechanics and muscle memory are developed through repetition. Perhaps some dry fire practice with the proper grip before the next trip is in order. Great excuse for another trip to the range with her.

I wasn't watching her hands. I should be more observant with her. Gives me more reason to get up close to her anyway. Good excuse to put my hands on her šŸ˜‰ hey I'm a regular Taran Butler huh? Just kidding
